The Rehabilitation Support Worker provides essential assistance to individuals undergoing rehabilitation for physical, mental, or emotional challenges. This role involves supporting patients in their daily activities, helping with therapeutic exercises, and working collaboratively with healthcare professionals to promote patient recovery and independence. The Rehabilitation Support Worker plays a crucial role in enhancing patients' quality of life by fostering a supportive and encouraging environment.
Duties and Responsibilities:
- Observe and monitor patients for changes in their condition, reporting any concerns or progress to the supervising therapist or nurse.
- Assist in taking and recording vital signs, such as temperature, pulse, respiration, and blood pressure.
- Help with routine health checks and support the administration of basic medical procedures as needed.
- Support patients in performing prescribed therapeutic exercises and activities, ensuring they are done safely and effectively.
- Encourage and motivate patients to actively participate in rehabilitation programs and work towards their recovery goals.
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting.
- Support patients with mobility tasks, such as transferring from bed to chair, walking, and using mobility aids.
- Accurately document patient care activities, progress, and any significant observations in patient records.
- Communicate effectively with patients, families, and the rehabilitation team to coordinate care and provide updates.
- Work closely with therapists, nurses, and other healthcare professionals to deliver integrated care and support to patients.
- Participate in team meetings to discuss patient progress, treatment plans, and any necessary adjustments to care.
Qualifications:
- High school diploma or equivalent.
- Previous experience in a rehabilitation or healthcare setting is preferred.
- Basic knowledge of rehabilitation techniques, exercises, and adaptive equipment.
- Strong communication and interpersonal skills.
- Compassionate and patient-centered approach to care.
- Ability to work effectively in a team environment.
- Physical stamina to assist with patient mobility and perform other physically demanding tasks.
